At Regional HealthPlus we strive to provide you, the patient, with superior medical services and an outstanding experience every time. Our CarePlus program is designed to provide quality, evidenced based medicine in the right setting, at the right time by helping both physicians and patients with care coordination.  It is easier said than done in the ever changing, and improving world of medicine.   We do this by providing your physician with the additional help of trained Care Navigators, Wellness Coaches, and Nurses.  These professionals work with you and your physician to help you reach your health goals.

CarePlus empowers patients through education and coaching.  CarePlus also encourages patients to take control of their health and the costs associated with healthcare.

CarePlus has programs available to promote prevention and wellness, chronic disease management, hospital inpatient care coordination, and help after hospital stays. The CarePlus model of care coordination has been recognized as a Cardiovascular Center of Excellence by Consortium of Southeastern Hypertension Control (COSEHC). COSEHC in particular provides evidence based guidelines and physician performance monitoring for chronic diseases such as high cholesterol, high blood pressure, and diabetes.
